Established in 2013 pursuant Minn. Stat. 256.478, the Transition to Community Initiative program (TTC) provides Minnesotans access to a range of services, including home and community based (HCBS) waivers, flexible grant funding, intensive care coordination, and partnerships with providers and counties to address an individual's unique needs. TTC promotes access to customized, community and integrated settings through a combination of county grants and housing/rental assistance. The incumbent in this position will work with TTC team members to provide direction and coordination for the Transition to Community (TTC) Initiative program expansion. Responsibilities include: Focus of the expansion is upon persons in state and community hospitals and CBHH's and ensuring access to services designed to facilitate discharges from those settings back to the community Address challenges and gaps in the mental health care continuum that lead people with mental illness to enter and remain in hospital settings Develop key, strategic partnerships with Central Pre-Admission, Forensic Mental Health Program, the Disability Services Division, the mental health innovations grant program team and other stakeholders Provide direction, coordination and technical assistance to the TTC program expansion work, including obtaining stakeholder participation in program development, management and monitoring the outcomes of the expansion project Work in cooperation with the data specialist, the employee will analyze community based and institutional data to develop possible diversion strategies Provide consultation, technical assistance, and mentoring to regional stakeholders Facilitate and coordinate continuous improvement activities with treatment providers that will result in increased collaboration and communication for a comprehensive mental health system of care
